Symbol + footprint reconstructed from manvalan/DigiRadio's EasyEDA project (same method as the FSC-BT1035 footprint), cross-checked against the manufacturer datasheet found in that same project. The datasheet's own circuit diagram shows two independent, mechanically -ganged contact pairs (not a simple SPST), reproduced faithfully in the symbol. Pin numbering preserved exactly as extracted, kept internally consistent between symbol and footprint.
